Our process for reviewing your Long-term Conditions in a single appointment
We have implemented a new single-appointment approach to managing a range of long-term conditions (see the list below). One or more of these conditions may often occur together for many patients. For example, if you have been diagnosed with both Diabetes and Heart Failure, you would only need to attend for one joint review, rather than two separate appointments. The goal is to offer a more joined up service and reduce the number of separate appointments that we invite you to attend.
We will contact you when it’s time for your review. This is an annual appointment with a Healthcare Assistant who will do things like take bloods, check your blood pressure, height and weight, and ask some questions regarding your condition management. The results of these tests are then reviewed by your usual GP, who will be in contact if needed.
Please be aware that the focus of these appointments is only to review the long-term condition(s) listed below that you have been diagnosed with. The time allocated to the appointment is based on the number of conditions that need review.
For discussion of other matters, including for example repeat prescriptions, acute healthcare needs, or any other queries relating to any other conditions you may have, please follow the usual procedures and contact us in the appropriate way. Please note, Healthcare Assistants are not qualified to prescribe medication.

- Bariatric surgery (weight loss surgery)
- Cardiovascular Disease – Primary Prevention
- Chronic Obstructive Pulmonoary Disease (COPD)

- Heart Failure

- Gout diagnosis AND you are prescribed Urate-Lowering Therapy i.e. Allopurinol or Febuxostat
- Hyperthyroidism (overactive thyroid)
- Hypothyroidism (underactive thyroid)
- Monoclonal Gammopathy of Unknown Significance (MGUS)
- Non-alcoholic Fatty Liver Disease (NAFLD), including nonalcholic steatohepatitis
