If you are able to, please contact us first online. It really helps us to manage our workload.
If you are unable to use our online systems for any reason, please just phone or visit your practice and we will make sure you are looked after.
askmyGP
Each Practice has a different askmyGP website:
Make sure you register and log in to askmyGP for the practice you are registered with.
Use askmyGP to:
- Complete an online form to ask about a non-urgent medical problem
- Ask a question relating to your healthcare
- Get information on self-care and community services
- Request a sick note
askmyGP should not be used for:
- medical emergencies. Please call 999 instead.
- repeat prescription requests. Please Use SystmOnline instead.
SystmOnline
Use your SystmOnline account to:
- Order a repeat prescription
- Get your test results
- View your medical record
To register for access to SystmOnline, please complete this Application for Access to SystmOnline.
If you are caring for someone, they can grant you ‘proxy access’ to SystmOnline, by completing this Proxy Access Consent Form.
Paper forms are available at your Practice. Once you have completed and signed the form, you will need to bring it to your practice along with the identification stated in the form.
Your NHS account
You can also use your NHS account (on the NHS website or in the NHS app) to:
- Order a repeat prescription
- Get your test results
- View your medical record
Unfortunately the staff at Mereside Medical practices are unable to provide technical support relating to your NHS account.
